Application Forms
For the Admissions Office to process your application, an applicant has to provide the following documents.
- A completed application form (Spanish Version)
- A copy of the applicant’s Birth Certificate
- A copy of the applicant’s Social Security Card
- Recommendation Information Form (Spanish Version)
- A completed “Consent to Release” form
(This form gives the student's current school permission to send records to Providence Cristo Rey High School. Parents are encouraged to urge current school principals to send transcripts, test scores, and other requested records as quickly as possible.)
- A copy of parents’ 1040 federal tax form and W-2 (s)
- Immunization records
- A non-refundable application fee of $10.00 (Payable by check made payable to Providence Cristo Rey High School, money order or credit card. Financial aid forms available from the Admissions Director)
Once all items have been turned in and the application materials have been reviewed by the Admissions Review Committee a decision will be made to grant the student an interview or deny the student admission.
If an interview is granted, the Office of Admissions will contact the applicant’s family to set up an interview time. For more information on the interview process, please click here.
